Primary Care Behavioral Health

Join Hartford Healthcare’s Primary Care Behavioral Health team, providing integrated mental health support within a collaborative primary care setting. In this role, you will conduct brief assessments, deliver evidence-based interventions, coordinate care with physicians and nurses, and support patients with depression, anxiety, and chronic disease management. You’ll document care in the EMR, participate in case conferences, and help develop care plans. Responsibilities

• Provide behavioral health assessments for primary care patients
• Deliver brief, evidence-based interventions (e.g., CBT, MI)
• Collaborate closely with primary care providers and care teams
• Develop and update patient-centered treatment and care plans
• Document all encounters in the EMR accurately and timely
• Participate in case conferences and interdisciplinary team meetings
• Support crisis assessment and intervention as needed
• Educate patients and families on mental health and wellness resources
• Coordinate referrals to specialty mental health and community resources
• Contribute to quality improvement and population health initiatives

Requirements

• Active state license in social work, counseling, psychology, or related behavioral health field (e.g., LCSW, LPC, LMFT, PsyD/PhD)
• Certification or formal training in evidence-based therapies (e.g., CBT, MI) preferred
• Prior experience in integrated primary care or collaborative care model preferred
