Job Description:
The ADON Palliative Performance Scale (PPS) Skilled Case Manager is responsible for coordinating and overseeing skilled nursing services for residents with palliative or complex medical needs. This role ensures that care plans align with each resident's clinical status, functional level, and goals of care as measured by the Palliative Performance Scale. The ADON PPS Skilled Case Manager promotes quality, comfort, and dignity through comprehensive assessments, care coordination, and interdisciplinary collaboration. The ADON PPS Skilled Case Manager serves as a clinical resource, ensuring residents receive appropriate skilled interventions while supporting families and staff through the continuum of care.
Essential Job Duties:

• Conducts comprehensive assessments of residents using the Palliative Performance Scale (PPS) to determine functional status, care needs, and progression.
• Develops, implements, and manages individualized care plans that align with each resident's palliative and skilled nursing needs.
• Coordinates and monitors skilled services, including nursing, therapy, and ancillary care, to ensure compliance with regulatory and payer requirements.
• Serves as liaison between physicians, nursing staff, therapy, social services, and families to ensure consistent communication and continuity of care.
• Reviews clinical documentation to ensure accurate PPS scoring and appropriate skilled service justification.
• Evaluates residents' response to treatment and adjusts care plans accordingly to meet changing clinical or palliative needs.
• Provides education to staff and families regarding palliative care goals, PPS scoring, and symptom management.
• Participates in interdisciplinary care conferences and quality assurance initiatives.
• Monitors and reports outcomes related to palliative and skilled care utilization.
• Ensures compliance with facility policies, clinical standards, and state and federal regulations.
• Performs other job-related duties as assigned to meet evolving facility needs.

Job Knowledge & Skills:

• Strong understanding of palliative care principles, symptom management, and the Palliative Performance Scale (PPS).
• In-depth knowledge of skilled nursing criteria, clinical documentation, and case management practices.
• Excellent assessment, critical thinking, and clinical judgment skills.
• Ability to interpret and apply regulatory and payer guidelines, including Medicare skilled criteria.
• Effective communication and collaboration skills for working with interdisciplinary teams, residents, and families.
• Strong organizational skills with attention to accuracy, timeliness, and detail.
• Compassionate approach to care coordination, focused on dignity, comfort, and resident-centered outcomes.

Qualifications & Education

• Current and active Registered Nurse (RN) license in the state of practice required.
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum of 3-5 years of clinical experience in skilled nursing, palliative care, hospice, or case management required.
• Demonstrated experience with care planning, PPS assessment, and interdisciplinary coordination preferred.
• Current CPR certification required.
• Working knowledge of electronic medical records (EMR) systems preferred.
• Must demonstrate a moderate level of computer literacy in order to use and access our systems and tools within our facilities to complete online tasks and documentation.
• Must be able to effectively communicate and interact professionally with staff and residents.

Physical Requirements:

• Ability to stand, walk, and move throughout the facility for prolonged periods.
• Must be able to lift, push, pull, and carry up to 20 pounds with one hand.
• Frequent bending, reaching, and assisting residents as needed.
• Visual and auditory acuity required for assessment, documentation, and communication.
• Ability to maintain physical and emotional stamina while working in a fast-paced, compassionate care environment.
