Post-Release Case Manager
Concordance's reentry program is designed to dramatically reduce recidivism by helping individuals with justice involvement lead meaningful, abundant, and joyful lives. The Post-Release Case Manager serves as a trusted guide, advocate, and accountability partner for participants by monitoring progress and helping participants navigate through obstacles. Post-Release Case Managers facilitate both individual and group sessions with participants and serve as the designated leader of participants' care teams. The Post-Release Case Manager collaborates with the care team to ensure participants are obtaining quality support as they advance through the program model.
