Sanford Health, one of the largest not-for-profit health systems in the US, is seeking a full-time or part-time Internal Medicine or Family Medicine physician to serve as a post-acute care and nursing home physician (SNF-ist). This role involves coordinating care across the continuum with a focus on reducing readmissions and enhancing patient experience within a comprehensive, integrated healthcare delivery system. The successful candidate will demonstrate a service-oriented mindset and be board certified or board eligible.

Responsibilities
- Coordinate care across the continuum
- Work collaboratively with nurse care managers, fellow Hospitalists, ER physicians, specialty and primary care physicians to form a plan of care for the patient
- Control readmission and improve the patient experience
- Perform history and physical examinations
- Admit (if applicable to the setting), evaluate, diagnosis, treat and provide consultative services to adolescent and adult patients with common and complex illness, diseases and functional disorders of the circulatory, respiratory, endocrine, metabolic, musculoskeletal, hematopoietic, gastroenteric, and genitourinary systems

Requirements
- PT or FT Internal Medicine or Family Medicine physician
- Service and access mindset
- Board certified/board eligible
- M.D. or D.O. degree
- Successful completion of an Internal Medicine residency program accredited by the ACGME, the American Osteopathic Association (AOA) or the Royal College of Physician and Surgeons of Canada
- Certification in the primary area of practice by the appropriate specialty/subspecialty board of the American Board of Medical Specialties (ABMS), the American Osteopathic Association (AOA) or the Royal College of Physicians and Surgeons of Canada, as applicable
- If not board certified at the time of application but completed residency training within the last five years, must achieve board certification within five years from the date of completion of residency training
