The Population Health Nurse supports high-risk patient populations through care coordination, outreach, and monitoring to improve health outcomes and reduce hospital utilization. This role includes local travel and may involve assisting patients with transportation to ensure access to care. This position may be filled by an RN or LPN based on licensure and experience, with responsibilities aligned to scope of practice.

Primary Responsibilities (All):

  • Identify and support high-risk patients requiring care coordination
  • Conduct patient outreach, follow-up, and education
  • Coordinate care across providers, departments, and community resources
  • Assist with transitions of care, including post-discharge follow-up
  • Monitor patient compliance with treatment plans and appointments
  • Support reduction of readmissions and unnecessary ED utilization
  • Assist patients with access to care, including coordinating and providing transportation when appropriate
  • Travel locally to support patient needs, appointments, and care coordination
  • Document patient interactions and care coordination activities
  • Collaborate with providers and staff to improve patient outcomes

Additional RN Responsibilities:

  • Perform patient assessments and identify care gaps
  • Develop and adjust care coordination plans
  • Interpret clinical information and elevate concerns
  • Collaborate directly with providers on care plans
  • Lead complex or high-risk patient cases
  • Provide advanced patient education and disease management support

Additional LPN Responsibilities:

  • Carry out established care coordination plans
  • Reinforce patient education and instructions
  • Monitor patient status and report changes to RN or provider
  • Perform routine follow-up calls and check-ins
  • Assist with scheduling, referrals, and documentation

Qualifications:

  • Licensed RN or LPN in the State of Iowa (or compact state)
  • Previous clinical, care coordination, or case management experience preferred
  • Strong communication and organizational skills
  • Ability to work independently and manage multiple patient cases
  • Valid driver’s license required

Compensation

Pay is based on licensure and experience using the clinic wage scale:

  • LPN: $23.38 – $35.00 per hour
  • RN: $28.71 – $39.00 per hour
