- Located in Central Massachusetts, practicing one of our premier partner skilled nursing facilities
- Hybrid work model, combining on-site care at the skilled nursing facility with the ability to complete certain patient care and administrative responsibilities remotely when appropriate
- Partner with an advanced practice clinician to manage patients based on goals of care: recovery of health, palliative or comfort care
- Lead multidisciplinary skilled rehab team including advanced practice clinician, nursing, dietary, social work, PT, OT, SLP, and case management
Responsibilities
- Provide value based care to patients on skilled rehab and long term care units
- Follow skilled therapy patients closely to track their progress and barriers
- Monitor and manage acute and chronic conditions common in nursing homes: CVA, progressive neurological conditions including Dementia (Alzheimer's and other Dementia diagnoses), Parkinson's Disease, MS, fractures, delirium and postoperative surgical patients, and end stage diseases
- Longitudinal follow-up and care of Long-Term Care patients residing at the facility per state regulations
- Discuss goals of care with patients and families and caregivers
Compensation
- A competitive compensation plan
- The salary range for this role is $204,500 to $352,000 annually based on full-time employment
- Salary Range is defined as total cash compensation at target
- The actual range and pay mix of base and bonus is variable based upon experience and metric achievement

Requirements
- National Board Certification in Internal Medicine or Family Practice
- Licensed by the Commonwealth of Massachusetts board of registration in Medicine
- Graduate of an Accredited Medical School
- Maintains BLS certification, professional certification as appropriate
- Preferred: Minimum two years of MD Experience
- Preferred: Geriatric experience
- Preferred: Geriatrics Fellowship
- Preferred: Hospice and Palliative Care Fellowship
- Preferred: Fluency in EMR systems and technology
