Humana's CenterWell Senior Primary Care is seeking a Physician Collaborator for a fully remote role. You will collaborate with APPs to support patients who recently fractured and require wraparound services, reporting to the Associate Director.
Applicants must hold an unrestricted medical license in KY, TN, MO, NC, TX, IN, be MD/DO trained, board certified or board eligible, and have 2+ years of experience collaborating with mid-levels. Occasional travel to Humana offices may be required.
