Qualifications

• Must be Board Certified OBGYN (Not Board Eligible) - Required
• Michigan License and CSR needed when you apply -
• BLS Certification - Required
• Provider must live locally - this is required, no exceptions
• Must have a clean background for quick credentialing - Required, no exceptions
• Provide your availability if you are not available on the listed dates - Required
• Verbal confirmation of DEA is needed at name clear
