This role leads post-discharge transitional care efforts, ensuring clinical stabilization, reducing preventable hospital utilization, and improving patient outcomes. Direct patient care is provided in a clinic setting with potential for home and community visits as needed.
Key Responsibilities
- Provide clinical care to complex, high-risk patients in a designated clinic, with periodic community visits when necessary.
- Lead post-discharge transitional care efforts, working closely with Primary Care Physicians and specialists.
- Participate in and lead interdisciplinary meetings to support patient care and ensure effective communication among healthcare providers.
- Oversee the clinical work of a High-Risk Nurse Practitioner conducting in-home visits.
- Refer patients to appropriate clinical, social, and behavioral health resources.
- Document all care provided in compliance with regulatory standards.
- Adhere to clinical guidelines, group protocols, and health plan requirements.
- Perform additional duties as assigned by management.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
- Board Certified/Board Eligible in Internal Medicine, Geriatrics, or Emergency Medicine (preferred but not required).
- Active California medical license and DEA privilege.
- Minimum of five years of clinical experience, with two years of managed care or health plan experience preferred.
- Proven excellence in both hospital-based and outpatient medical practices.
- Strong clinical knowledge, communication, and organizational skills.
- Ability to work in a fast-paced environment, independently and collaboratively.
- High personal ethics and the ability to build positive relationships with management, medical directors, and department heads.
- Salary: $270,000 - $300,000 annually, based on experience, skills, and location.
