Highlights
Community Physician Network is seeking an Internal Medicine physician or Endocrinologist with special interest in Type-II Diabetes management to join our new Diabetes Care Clinic in Kokomo, Indiana.
The physician in this role will join a multi-disciplinary team (NP, pharmacy, social work, Diabetes educator) in an outpatient setting. Ideal candidates will be collaborative and have a passion for educating patients to improve outcomes and quality of life.
- Outpatient-only position

Licensure & Certifications
- BE/BC in Internal Medicine or Endocrinology
- Additional experience/certification in Diabetes care
- Licensed or eligible for medical licensure in Indiana
