PhysicianWCHC Family Medicine - Washington, IA 52353OverviewPosition Type Full Time Job Shift Day Category Health CareDescriptionThe Physician provides comprehensive, patient-centered medical care within their scope of training and specialty in a clinic and/or hospital setting. This role may include primary care or specialty services depending on assignment. This Physician is responsible for evaluating, diagnosing, and treating patients while promoting preventive care, managing acute and chronic conditions, and coordinating care across the healthcare continuum.

The requirements listed below are representative of the knowledge, skill, and/or ability to be successful.DUTIES/RESPONSIBILITIES (include but are not limited to the following)Provide medical care appropriate to specialty, including preventive, acute, and/or chronic disease managementConduct patient assessments, histories, and physical examinationsDiagnose and treat a range of conditions within scope of practiceOrder, interpret, and communicate laboratory and diagnostic test resultsPerform procedures appropriate to specialty and credentialingPrescribe medications and develop treatment plansManage chronic conditions such as diabetes, hypertension, COPD, and heart diseaseProvide age- and condition-appropriate screenings and preventive servicesCoordinate care with other physicians, specialists, hospitals, and care providersFacilitate referrals and ensure appropriate follow-up careParticipate in transitions of care and population health initiatives as applicableCollaborate with interdisciplinary teams to optimize patient outcomesEducate patients and families on diagnoses, treatment plans, and prevention strategiesFoster a patient-centered environment that promotes understanding and trustBuild and maintain therapeutic relationships with patientsMaintain accurate, timely, and complete electronic medical recordsDocument patient encounters, care plans, and outcomesComply with regulatory, billing, and quality reporting requirementsReview and sign charts per organizational standardsParticipate in community health initiatives and outreach as appropriateSupport organizational quality improvement and performance initiativesQualificationsEDUCATION AND/OR EXPERIENCEDoctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)Board Certified or Board Eligible in Family Medicine or Internal MedicineActive Iowa medical license (or ability to obtain)Current DEA and CSA licensesCurrent BLS/ACLS certificationNPI number and eligibility for Medicare/Medicaid enrollmentCredentialing and privileging through WCHCSKILLS AND ABILITIESStrong clinical decision-making and diagnostic skillsExcellent interpersonal and communication skillsAbility to practice within a defined specialty or broad scope of careEffective time management and organizational skillsAbility to work independently and within a team-based care modelAdaptability in a rural healthcare environment
