Qualifications

• MD or DO with Board Eligibility/Certification in Hematology/Oncology.
• Current and unrestricted state and federal authority to prescribe and administer medications appropriate to the scope of practice.
• Active DEA registration.
• Favorable malpractice history and clean background (successful self‑query through the NPDB).
