Physician - Family Medicine
Job Details:

• Permanent, full-time outpatient Family Medicine opportunity serving a diverse, underserved patient population of all ages
• 100% outpatient position with no inpatient responsibilities
• Predictable Monday through Friday schedule, 8:00 AM to 5:00 PM
• Approximately 20 patients per day with a mix of pediatric (30%) and adult patients
• Clinical allocation of 1.5 days per week Pediatrics and 3.5 days per week Adult Medicine
• Provide comprehensive primary care services including preventive health screenings, routine physicals, and wellness visits
• Diagnose, treat, and manage acute and chronic medical conditions across all age groups
• Deliver pediatric primary care including well-child visits, immunizations, and treatment of common pediatric illnesses
• Develop individualized chronic disease management and treatment plans
• Educate patients and families on disease prevention, health maintenance, and treatment plans
• Coordinate care with specialists, ancillary services, and interdisciplinary healthcare teams
• Maintain accurate and timely documentation within Epic EMR
• Participate in limited phone/beeper call coverage, approximately one week annually
• Support quality improvement initiatives related to patient outcomes and access to care
• Deliver culturally sensitive and patient-centered care to the community
• Board Certification in Family Medicine required
• Active DEA certification and state medical license required at time of submission
• Minimum of one (1) year of physician experience required
• Clean malpractice and license history highly preferred
• At least one professional reference within the past 24 months required
