Your work will include:Conducting comprehensive geriatric assessments, including medical history, physical examination, functional status, cognitive health, and medication reviewDeveloping individualized care plans based on each patient’s medical needs, personal goals, and quality-of-life prioritiesManaging complex chronic illnesses and multiple comorbidities common among older adultsEvaluating medication regimens and helping reduce unnecessary or potentially inappropriate medicationsCollaborating with nursing, advanced practice providers, rehabilitation professionals, specialists, social services, and other members of the care teamPartnering with patients, families, and caregivers to support informed decision-making and care planningProviding education regarding disease management, healthy aging, preventive care, and available resourcesCoordinating care across specialties and healthcare settings to promote continuityParticipating in interdisciplinary meetings and quality-improvement initiativesApplying current evidence-based practices in geriatrics and long-term care medicineWho We’re Looking ForWe are looking for a physician who is passionate about caring for older adults and values the relationships that are central to geriatric medicine.Qualifications include:MD or DO from an accredited medical schoolBoard Certified or Board Eligible in Internal Medicine, or Family MedicineFellowship training in Geriatric Medicine preferredEligible for an unrestricted Maine medical licenseExcellent communication and interpersonal skillsAbility to work collaboratively within an interdisciplinary healthcare teamCommitment to compassionate, patient-centered care and shared decision-makingExperience with Epic or other electronic health record systems preferredMore Than a Job—An Opportunity to Practice Medicine with PurposeFor physicians drawn to geriatrics, some of the most rewarding moments in medicine come from helping patients and families navigate complex decisions, preserve independence, manage multiple health conditions, and focus care on what matters most.At St.
