Reducing hospital sick days Primary Care Outpatient Low patient volume - 15-18 patients per day Value Based Care Model Patient population: Seniors with 3-5 complex diseases (Hearth disease, diabetes, hypertension etc.) Benefits: Health, Vision, Dental (including family options) & paid parental leave program Pre-tax or post-tax Roth contributions available.
