Geriatrician Provider Duties
Conduct comprehensive geriatric assessments for elderly patients, including physical, cognitive, emotional, and functional evaluations.
Diagnose and manage acute and chronic medical conditions specific to older adults.
Develop individualized care plans in collaboration with patients, families, and interdisciplinary teams.
Review, reconcile, and optimize medication regimens to minimize polypharmacy and adverse drug events.
Provide consultative or primary outpatient, inpatient, and/or long-term care to geriatric patients.
Assess and manage neurocognitive disorders, including dementia, delirium, and depression.
Coordinate patient care with other healthcare providers, specialists, therapists, pharmacists, and social workers.
Evaluate and address functional decline, falls, mobility issues, and frailty in elderly patients.
Facilitate referrals to specialty services, community agencies, rehabilitation programs, and support resources.
Conduct discussions regarding prognosis, goals of care, advance care planning, and advance directives with patients and families.
Provide education and counseling to patients and caregivers on disease management and preventive care.
Document all patient encounters and care plans accurately and timely in the electronic health record (EHR).
Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives.
Ensure compliance with facility policies, procedures, and accreditation and regulatory requirements.
Provide teaching and supervision to medical students, residents, fellows, and other healthcare staff regarding geriatric medicine.
Engage in continuing medical education and stay current on best practices and guidelines in geriatric care.
Participate in research and scholarly activities as appropriate.
Perform additional clinical and administrative duties as assigned by leadership.
