Signify Health is seeking a mission-driven Physician Assistant to perform comprehensive in-home clinical evaluations for diverse, often vulnerable patients. In this field-based role, you will conduct detailed histories and physicals, review medications, identify care gaps, and document findings in our EHR to support value-based care programs. You’ll collaborate with interdisciplinary teams, use data and analytics to inform care recommendations, and educate patients and caregivers on conditions and preventive care.
