Position Summary:The Nurse Practitioner (NP) or Physician Assistant (PA) provides diagnostic, therapeutic, and preventive healthcare services to patients across the lifespan. Working collaboratively with physicians and other healthcare team members, the NP/PA obtains medical histories, examines and treats patients, diagnoses illnesses and injuries, orders and interprets diagnostic testing, prescribes medications, and provides patient and family education.This position provides care in the clinic, hospital, and Emergency Department settings. Responsibilities include managing minor injuries, performing approved procedures such as suturing, splinting, and casting, and conducting emergency medical screening examinations and interventions within the provider’s authorized scope of practice and clinical privileges.Essential Job Functions:Patient Assessment and Clinical CareProvide high-quality, age-appropriate care in the clinic, hospital, and Emergency Department.Obtain medical histories, perform physical examinations, and assess patients’ healthcare needs.Diagnose and treat acute and chronic illnesses and injuries.Develop, implement, and evaluate individualized treatment plans.Order and interpret laboratory testing, diagnostic imaging, and other clinical studies.Prescribe and manage medications in accordance with applicable laws and organizational policies.Perform approved procedures, including suturing, splinting, casting, and treatment of minor injuries.Provide medical screening examinations in the Emergency Department as authorized.Care Coordination and Patient EducationCollaborate with physicians and other healthcare professionals to provide coordinated patient care.Consult with and refer patients to specialists and community resources when appropriate.Educate patients and families regarding diagnoses, treatment plans, medications, follow-up care, and preventive health practices.Promote wellness, illness prevention, and effective disease management.Documentation and ComplianceMaintain accurate, complete, and timely medical records and required documentation.Protect patient confidentiality and comply with HIPAA requirements.Follow applicable federal and state regulations, professional standards, and organizational policies.Comply with infection prevention, medication safety, and workplace safety requirements.Quality and Professional PracticeEvaluate patient care processes and outcomes to support safe, effective, and cost-conscious care.Participate in quality improvement, peer review, staff education, and community health initiatives.Maintain required licenses, certifications, continuing education, and professional competencies.Support the mission and goals of HCHS and promote Heartland Family Medicine Rural Health Clinic within the community.Perform other duties consistent with the position’s responsibilities and scope of practice.RequiredGraduate of an accredited Nurse Practitioner or Physician Assistant educational program.Current and unrestricted Nebraska license to practice as a Nurse Practitioner or Physician Assistant.BLS, ACLS, PALS & ATLSPreferredMaster’s degree in nursing, physician assistant studies, or a related clinical field.Minimum of two years of clinical experience.Core CompetenciesStrong clinical judgment, assessment, and diagnostic skillsExcellent interpersonal and communication skillsStrong patient education and counseling abilitiesProfessionalism, discretion, and sound judgmentHigh level of accuracy and attention to detailAbility to work independently and collaboratively within a healthcare teamWorking ConditionsWork is performed in clinic, hospital, and Emergency Department settings and may involve exposure to infectious diseases, bodily fluids, and other healthcare-related hazards.
