Position Summary

Responsibilities

  • Coordinate and deliver primary care in collaboration with the Practice Site Physician by: Working within their approved scope of practice; obtaining complete medical history and physical data on patients; interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures as needed; performing a complete physical exam and recording findings; collaborating with physicians in managing the medical needs of patients; ordering appropriate laboratory and diagnostic procedures; educating and advising patients regarding health and illness prevention; recommending community resources to meet patient and family needs; supporting patient access and care coordination through triage, follow-up, and management of urgent situations.
  • Provide clinical and administrative assistance to BMG programs, e.g., education, follow-up, and related activities by: participating in health education; maintaining related records and statistical data and preparing statistical reports as needed; participating in interdisciplinary care conferences and assisting with referrals and dissemination of information to appropriate vendors; assisting in the development and coordination of ongoing education and follow-up programs designed to measure client outcomes.

Organizational Responsibilities

  • Attends and participates in department meetings and is accountable for all information shared.
  • Completes mandatory education, annual competencies, and department-specific education within established timeframes.
  • Completes annual employee health requirements within established timeframes.
  • Maintains license/certification, registration in good standing throughout the fiscal year.
  • Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.
  • Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patients and self.
  • Adheres to regulatory agency requirements, survey process and compliance.
  • Complies with established organization and department policies.
  • Available to work overtime and additional shifts and schedules when required.

Education / Licensure / Certification

  • Graduate of an accredited Physician Assistant or Nurse Practitioner program.
  • Current, valid state licensure and appropriate certification (NCCPA or ANCC/AANP) obtained prior to hire date or job transfer date. Verified by local Credentialing Office.
  • BLS obtained within 1 month (30 days) of hire date or job transfer date if required. American Heart Association or American Red Cross accepted.
  • Current BLS and ACLS certifications if required.
  • Ability to work collaboratively in a fast-paced, team-oriented environment.

Knowledge & Skills

  • Requires a thorough knowledge of practice standards and policies and procedures of BMG and the appropriate professional organization.
  • Requires a high level of analytical skills necessary to assess the patient and participate in follow-up care planning.
  • Demonstrates interpersonal skills necessary to effectively interact with patients, families, physicians, other professionals, and paraprofessionals.
  • Demonstrates a high level of communication skills, both oral and written, necessary for effective communication with patient/family, physicians, and referral sources.

Working Environment And Physical Demands

  • Works in a clinic environment requiring physical exertion, frequent changes in job demands, and exposure to biohazards.
  • Requires the physical ability and stamina to walk/stand for prolonged periods, push carts/wheelchairs, lift up to 50 pounds, position/lift patients at a maximum of 35 pounds without assistance, provide CPR, etc.
