Signify Health seeks a Physician Assistant to perform comprehensive in-home health evaluations for diverse patient populations. You will obtain histories, conduct physical exams, order and interpret diagnostics, and document findings in EMR to support value-based care programs. Collaborating with physicians, nurses, and care coordinators, you’ll identify gaps in care, reconcile medications, and educate patients on chronic disease management.
