Physician Assistant - Health Equity for Adolescents in the Legal System (HEAL)

Position Overview

Physician Affiliate Group of New York (PAGNY) invites a full‑time Physician Assistant to join the Health Equity for Adolescents in the Legal System (HEAL) program in partnership with the Administration for Children’s Services (ACS). The position delivers 24/7 medical care to adolescents in secure detention sites, school‑based health centers, and non‑secure group homes, providing comprehensive medical services and referrals.

Key Responsibilities

• Work locations at Crossroads Juvenile Center and/or Horizon Juvenile Center.
• 36‑hour work week: Day, Overnight, and Weekend shifts.
• Provide medical care to ACS adolescents while maintaining safety and security protocols.
• Take relevant patient history, identify patient needs, and make diagnoses.
• Develop and implement treatment plans using diagnostic data.
• Perform complete well‑care examinations, including physical examinations.
• Perform complete gynecological histories and physical examinations.
• Write prescriptions, inform patients about medications prescribed; obtain parent/guardian consent when needed.
• Complete medical forms and documentation related to patient care.
• Make and track all referrals to other agencies and providers, with follow‑up care provided.
• Assist with phlebotomy, as needed, to ensure smooth patient flow.
• Administer immunizations, as needed.
• Distribute, refill, discard, and discontinue medication for patients.
• Complete Court Orders and all other ACS‑mandated paperwork.
• Evaluate and treat ACS employees (ONLY on an emergent basis).
• Provide health education on topics including preventive medicine and counseling on chronic diseases, parenting, prenatal, nutrition, family‑planning, medication usage, and stress management.
• Administrative duties:
  • Act as on‑site supervisor of Patient Care Associates while in the field.
  • Teach PCAs about patient care and triage to improve patient flow, and communicate with the Director of Nursing regarding status of flow at clinic site.
  • Participate in case conferences in which patient care is discussed.
  • Assist the Medical Director in the development and implementation of medical protocols and medical forms.
  • May initiate and participate in research related to health care or allied fields, in collaboration with the Medical Director.

Qualifications

• Current New York State Physician Assistant registration.
• Current certification issued by the NCCPA.
• Experience in acute care, college health, community health, family health, holistic care, pediatrics, psychiatry, school health, and/or women’s health.
• Must pass all appropriate screenings to work in a secured setting with young people.
• Spanish language proficiency preferred (not required).

Wages & Benefits

Annual Base Salary: $131,127 - $174,836 based on a 36‑hour work week and years of State licensure.

The estimated total value of the compensation package is $144,239 - $192,319, which includes the baseline salary, 401(k) contribution, and other factors.

• 401(k) company contribution (subject to IRS limits):
  • Employees are immediately vested in a 3% company contribution of base earnings. No employee match is required.
