Role Overview:
The NP/PA Clinician in Physical Medicine and Rehabilitation (PM&R) provides specialized rehabilitative services at skilled nursing facilities (SNFs) or post-acute rehab centers. They collaborate with interdisciplinary teams to optimize patient outcomes, manage conditions like CVA and neurological issues, and promote patient independence. The position emphasizes clinical leadership, teamwork, and continuous improvement.
Key Responsibilities
- Deliver PM&R services and represent the organization within the facility.
- Work closely with therapy teams to assess progress and address barriers to therapy.
- Manage conditions common in SNFs, such as neurological and orthopedic issues.
- Participate in multidisciplinary meetings and culture-building activities.
Qualifications & Skills
- Nurse Practitioner, APRN, or Physician Assistant with Arizona license.
- Experience in post-acute or SNF settings preferred.
- Autonomous, compassionate, and eager for growth.
- Strong interpersonal skills and commitment to high-quality patient care.
