Signify Health is seeking a Physician Assistant for in-home visits to deliver high-quality clinical evaluations to vulnerable patients. In this role, you will travel to patient homes in the Dallas area, performing comprehensive health assessments, documenting findings in EHR systems, and identifying care gaps and risk factors. You will collaborate with physicians, care coordinators, and health plans to support value-based care, educate patients and caregivers, and connect them with needed services.
