Signify Health is seeking a Physician Assistant to perform in-home clinical evaluations, focusing on vulnerable and chronically ill populations. You will conduct comprehensive health assessments, review medical histories, perform physical exams, order and interpret basic diagnostics, and document findings in EMR systems to support value-based care programs. Collaborating with interdisciplinary teams, you’ll help close care gaps, optimize risk adjustment, and connect patients with appropriate follow-up services.
