Job Description
Stamford Health Medical Group is seeking a driven& self-starter Nurse Practitioneror Physician Assistant to join our well-established and heavily growing Diabetes & Endocrinology Team! In this 100%-based office position, the APRN or PA will closely work alongside our seven Endocrinologists & Diabetes physicians and play a crucial role in providing comprehensive care to patients with endocrine disorders, including diabetes, thyroid conditions, and other hormone-related issues to provide a broad spectrum of patient care, including evaluating & educating patients, reviewing labs, triaging patient phone calls, & overseeing medication management.
Primary Location: 260 Long Ridge Road Stamford, CT with occasional rotation to secondary location
Work Schedule: Fulltime 40 hours per week; Monday through Friday 8a-5p; no weekends/holidays/call required
Major Responsibilities:
Conduct thorough patient assessments. Approximately 8-12 patients per day.
Evaluate and educate patients about diabetes and weight management.
Order and interpret diagnostic tests
Develop accurate diagnoses and treatment plans
Prescribe medication and administer treatments
Collaborate with the interdisciplinary team for seamless coordination of care
