Job Title

Nurse Practitioner / Physician Assistant – Primary Care (Family Medicine)

Location

Medford, The Dalles, McMinnville, Lake Oswego, Albany, Grants Pass, Eugene and Redmond, Oregon

Compensation

Base pay range: $120,000–$160,000 per year

Additional compensation: Guaranteed salary with earning potential of $300k+

Responsibilities & Duties

  • Provide direct patient care through office visits, telehealth, and occasional same-day appointments.
  • Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions.
  • Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions.
  • Educate and empower patients on preventive health and functional medicine principles.
  • Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff.
  • Document visits in the EHR (NextGen) in a timely and compliant manner.
  • Participate in team meetings, quality improvement initiatives, and training sessions.
  • Support clinic operations by contributing to a positive, team-oriented environment.

Required Qualifications

  • Active Oregon license as a Nurse Practitioner (FNP, AGNP, or AGNP-PC), PA-C, MD, or DO (or eligibility to obtain).
  • Board certification in Family Medicine, Internal Medicine, or relevant specialty.
  • Minimum 2 years of clinical experience (functional/integrative medicine experience a plus).
  • Strong interest in preventive, functional, and lifestyle medicine.
  • Excellent communication and patient relationship skills.
  • Comfortable working in a fast-paced, collaborative environment.
  • Proficient with electronic health records (NextGen experience a plus).
