Signify Health seeks a Physician Assistant to perform comprehensive in-home health evaluations for Medicare and other vulnerable populations. You will conduct histories and physicals, review medications, order/interpret basic diagnostics, and document findings in our digital platform to support value-based care. Collaborating with interdisciplinary teams, you’ll identify care gaps, educate patients, and coordinate follow-up with primary and specialty providers.
