The Physical Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.

Responsibilities
- Assess and evaluate patient care needs related to functional status, mobility, endurance, stamina, pain, and other conditions.
- Determine treatment plan based on assessment and evaluation.
- Perform interventions to improve and enhance patients' well-being.
- Evaluate patient progress.
Qualifications
- Bachelor's degree in a health-related field such as biology, exercise science, or kinesiology.
- Graduate of an accredited program from a CAPTE-accredited school.
- No experience required.
Licenses and Certifications
- PT – Physical Therapist upon hire.
- CPR – Cardiac Pulmonary Resuscitation upon hire.
- Driver License – Current driver's license and vehicle insurance, and access to a dependable vehicle.
