- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License is Required to Start the Assignment
- BLS Certification May Be Required from AHA or ARC
- Home health experience

- Coverage area includes the central Thumb region, including Cass City, Kingston, Mayville, Marlette, and Deford
