Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join a home health team in Alpena, MI as a traveling Physical Therapist. Provide skilled evaluations, therapeutic interventions, and discharge planning for patients recovering from injury or managing chronic conditions in home settings. You will work with a multidisciplinary team and use HCHB electronic medical records to document care and communicate with clinicians and families.

Minimum Qualifications

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Michigan Physical Therapy license required to start the assignment.
• At least 12 months of home health physical therapy experience within the past 3 years (client-specific requirement).
• Experience with HCHB (Homecare Homebase) electronic medical record required (client-specific requirement).
• BLS certification from AHA or ARC may be required.
• Valid driver’s license and reliable transportation to travel to patient homes within the assigned territory.

Preferred Qualifications

• Prior travel or per-diem home health experience.
• Experience managing caseloads across multiple ZIP codes within a rural service area.

Assignment Details

• Shift schedule: Monday through Friday, 40 hours per week.
• Daily shift: 8 hours per day.
• Territory includes ZIP codes: 49707, 49777, 49766, 49747, 49744, 49753, 49766, 48742, 48721, 48740.

Provide skilled, individualized home-based physical therapy that improves functional independence and safety. Collaborate with nurses, occupational therapists, social workers, and family caregivers to develop and execute patient-centered plans of care. Accurately document clinical progress in HCHB to support outcomes and continuity of care.
