VitalCaring seeks a Physical Therapist to provide compassionate, evidence-based care to home health and hospice patients. In this field-based role, you will evaluate patients, develop and deliver individualized treatment plans, and help improve mobility, safety, and quality of life at home. You will collaborate closely with an interdisciplinary team, document care in our EMR, and support patients and caregivers through education and coaching.
