The Emerald Peek Rehabilitation and Nursing Center seeks a licensed Physical Therapist to provide skilled therapy services in our resident-centered skilled nursing and short-term rehab facility in Peekskill, NY. The PT will complete comprehensive evaluations, design and implement individualized treatment plans, and support residents in improving mobility, function, and quality of life across subacute rehab, long-term care, and memory care programs. Working within a compassionate, interdisciplinary team, the therapist will document care thoroughly, educate residents and families, participate in care conferences, and uphold high standards of clinical excellence, safety, and dignity.
