The Vineyards Healthcare Center, a high-performing 83-bed nursing home in Livermore, is seeking a dedicated Physical Therapist to join its team.

Responsibilities

1. Conducting patient evaluations.
2. Developing treatment plans.
3. Monitoring progress.
4. Educating patients and caregivers.

Qualifications

1. Physical Therapy degree.
2. State licensure.
3. Strong interest in geriatric care.
4. New graduates are welcome; experience in skilled nursing is a plus.
5. Strong communication and problem-solving skills.
6. EMR proficiency is desired.
