St. James Rehabilitation and Healthcare Center seeks a full-time Physical Therapist to provide person-centered rehab services in our skilled nursing facility on Long Island. You will complete comprehensive PT evaluations, design and deliver individualized treatment plans, and focus on maximizing residents’ mobility, function, safety, and quality of life.
Collaborating with an interdisciplinary team, you will participate in care planning, provide family education, document outcomes, and support our culture of compassionate care, teamwork, and professional growth.
