Post Rehab is seeking a Physical Therapist (PT) to join the rehabilitation team at Civita Care Center at Long Ridge. This full-time opportunity allows you to work directly with residents to improve movement, restore strength, and build greater independence while collaborating with an experienced post-acute care team.
What You'll Do
- Evaluate residents and develop individualized physical therapy plans of care
- Provide skilled treatment addressing mobility, gait, balance, strength, transfers, and endurance
- Establish functional rehabilitation goals and track resident progress
- Adjust treatment approaches based on resident needs and clinical response
- Educate residents and caregivers on exercises, mobility strategies, and safety techniques
- Maintain accurate and timely clinical documentation
- Partner with PTAs, OT, SLP, nursing, and other members of the interdisciplinary team
- $45-$50/hour

- Graduate of an accredited Physical Therapy program
- Strong clinical, communication, and organizational skills
- Skilled nursing or rehabilitation experience preferred, but not required
