The Program of All-Inclusive Care for the Elderly (PACE)is one of the most rewarding settings in rehabilitation. As a PT in our PACEprogram, you will build long-term therapeutic relationships with elderlyparticipants, helping them maintain independence and remain in theircommunities. Unlike acute or post-acute settings, PACE allows you to followpatients over time, see the cumulative impact of your interventions, and trulyget to know the people you serve.
What You Will Do
You will provide comprehensive physical therapyevaluations and treatments for PACE participants in a community-based setting.Your work will focus on fall prevention, mobility optimization, painmanagement, and functional maintenance. You will collaborate with aninterdisciplinary team including physicians, nurses, social workers, andoccupational therapists to develop holistic care plans. The pace of work issteady and sustainable, with an emphasis on quality over volume.

What You Bring
• DPT or Master's degree from an accredited PT program
• Current or eligible for Pennsylvania State PT license
