Physical Therapist - Home Health
Schedule
Self-scheduled visits with minimal 6 skilled home care visits per day and 1-2 weekends per contract.
Assignment Details
Contract length and guaranteed hours are based on the needs of the HomeCare office.
Requirements

• 1 year of home health experience required
• Valid driver's license and personal vehicle required for home visits
• BLS certification required
• State license required
• OASIS training preferred
• COVID-19 vaccination required or exemption form on file
• Flu shot or declination required
• Mandatory mask wearing at all times while at work

Responsibilities

• Conduct patient assessments in cardiopulmonary/circulatory areas
• Document treatment plans and patient progress
• Utilize equipment and devices, including gait belts and walkers
• Educate patients and families on exercises and safety measures
• Monitor signs and symptoms of physical and psychological conditions
• Ensure safety and injury prevention during home visits
