Post Rehabilitation Services is hiring a Physical Therapist at Civita Care Center at Cheshire. You'll help residents regain mobility after illness or injury and address ongoing physical challenges through care tailored to their daily needs.
What You’ll Do
- Evaluate residents' movement and establish functional treatment goals
- Provide therapy to improve strength, endurance, balance, and walking
- Address transfer safety and recommend appropriate mobility devices
- Reassess residents and update care plans as their abilities change
- Document outcomes and coordinate recommendations with nursing and the rehab team

Qualifications
