Position Responsibilities
Evaluate residents and develop individualized treatment plans
Provide skilled physical therapy services
Monitor and document resident progress
Collaborate with nursing, rehabilitation, and interdisciplinary teams
Help residents improve mobility, strength, function, and independence
Qualifications
Graduate of an accredited Physical Therapy program
Eligible for Physical Therapist licensure in Vermont
Strong clinical, communication, and patient-care skills
SNF/LTC experience is welcome but not required
