Title: Physical Therapist
Duration: 3+ months + Possibility of Extension
Location: Ponca City, OK 74604
Coverage Areas: Fifty Miles East, West and South, North to Kansas Border
Shift: M-F (8 AM - 4:30 PM)
Roles & Responsibilities:
Conduct comprehensive physical therapy evaluations in patients' homes
Assess functional mobility, balance, strength, range of motion, and rehabilitation needs
Develop individualized physical therapy treatment plans based on physician orders and patient goals
Establish measurable outcomes and monitor patient progress throughout care
Assess home environments and recommend modifications to improve safety and accessibility
Provide clinical direction and oversight to Physical Therapist Assistants (PTAs)
Conduct supervisory visits in accordance with state regulations and agency policies
Minimum Qualifications:
Active and unrestricted Physical Therapist (PT) license
Current BLS (Basic Life Support) Certification or ability to obtain prior to seeing patients
Valid driver's license
Current automobile insurance
Preferred Qualifications:
Previous Home Health Physical Therapy experience
Experience with Homecare Homebase (HCHB) and/or PointCare
Strong knowledge of rehabilitation principles and functional mobility training
Experience with wheelchair assessments and durable medical equipment recommendations
Excellent communication and patient education skills
