Homeside Rehab is seeking licensed Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to provide home-based therapy services to patients in Borough Park, Brooklyn , specifically within the 11219 and 11204 ZIP codes.
This is an excellent opportunity for therapists looking to add flexible, local home care cases to their existing schedules while working with a supportive clinical and administrative team.
Responsibilities
Provide skilled physical therapy services in the patient's home
Evaluate patient needs and develop appropriate treatment plans, as applicable to the PT role
Provide treatment according to the established plan of care
PTAs will provide treatment under the supervision of a licensed Physical Therapist
Monitor patient progress and communicate changes to the clinical team
Educate patients and caregivers regarding treatment plans and home exercise programs
Complete accurate and timely clinical documentation
Maintain professional communication with patients, caregivers, and the Homeside Rehab team
Qualifications
Active New York State Physical Therapist (PT) or Physical Therapist Assistant (PTA) license
Minimum 1 year of experience preferred
Home care experience preferred, but not required
Reliable transportation
Strong communication and documentation skills
Ability to work independently in a home care setting
Dependable and able to maintain a consistent patient visit schedule
