Role Overview: This position involves evaluating and treating patients in Rehab and LTAC hospitals, documenting patient progress in the EMR, educating patients, families, and staff, and communicating with healthcare team members and outside professionals.
Key Responsibilities:
- Performing initial and discharge evaluations, setting treatment goals, and documenting appropriately.
- Developing and implementing effective treatment plans to achieve functional goals.
- Assessing therapy educational needs and providing PRN therapy as needed.
- Participating as a collaborative member of the interdisciplinary team.
- Serving as a clinical instructor/mentor for interns when required.
- Engaging in hospital and discipline-specific skills assessments.
- Managing equipment orders and liaising with vendors and external physicians.
Qualifications & Skills:
- Bachelor's Degree in Physical Therapy required; Doctoral Degree preferred.
- Minimum of 1 year experience in Rehab, SNF, LTACH, or Acute care preferred.
- Valid BLS certification and GA PT licensure or eligibility.
