CenterWell Home Health seeks a compassionate Physical Therapist Assistant to provide in-home rehabilitative care. In this role, you’ll implement treatment plans under the direction of a licensed Physical Therapist, assist with therapeutic exercises, gait and balance training, and patient education to promote safety, independence, and improved quality of life. You’ll collaborate closely with an interdisciplinary care team, document patient progress, and communicate changes in condition.
