Phoenix of Maple Heights is hiring a Full-Time Physical Therapist (PT) to join our dedicated rehabilitation team in Maple Heights, Ohio!

What You'll Do

• Evaluate residents and develop individualized treatment plans

• Provide skilled physical therapy services in a SNF/LTC setting

• Help residents improve strength, balance, mobility, transfers, and functional independence

• Monitor progress and modify treatment plans as needed

• Collaborate with nursing, therapy, physicians, and the interdisciplinary care team

• Participate in care planning and discharge planning

• Complete accurate and timely documentation using Net Health and PointClickCare (PCC)

• Provide compassionate, resident-centered care

What We’re Looking For

• Graduate of an accredited Physical Therapy program

• Current or eligible Ohio Physical Therapist license

• Strong clinical, communication, and interpersonal skills

• Team-oriented approach to resident care

• SNF/LTC experience is a plus
