Job Overview:
A dedicatedPhysical Therapist is needed to join an interdisciplinary team (IDT) focused on providing high-quality, patient-centered care. This role is crucial incoordinating and delivering rehabilitative services to enhanceparticipant independence and mobility , whether at thecenter or in-home settings .
Key Responsibilities:
- Conductcomprehensive Physical Therapy assessments in both facility and home environments.
- Develop and implementcustomized care plans in collaboration with an interdisciplinary team.
- Provideactive and passive therapy interventions , includingmuscle re-education, gait training, and ADLs (Activities of Daily Living) .
- Utilize treatment methods such asmassage, heat, sound, water, light, or electrical therapy .
- SuperviseTherapy Assistants , offering training and guidance forFunctional Maintenance Programs (FMP) .
- Educatestaff, caregivers, and family members on therapeutic equipment usage and patient mobility strategies.
Qualifications:
- Bachelor’s Degree in Physical Therapy with at least5 years of experience .
- Master’s or Doctorate in Physical Therapy (Preferred).
- Valid California Physical Therapy License or eligibility to obtain one.
- Minimum of 2 years of experience in a clinical setting, preferably working with frail or elderly populations.
- CPR certification preferred.
- If educated outside the U.S., the Physical Therapy degree must be evaluated for U.S. equivalency.
Compensation & Benefits:
- Base Salary: $100,000 - $120,000 (DOE)
