Physical Therapist

REQUIREMENTS:

• 1. CPR Certification (required).
• 2. Licensed Physical Therapy Therapist (required).
• 3. 1-2 years experience in long term care preferred.
• 4. Good physical and mental health.
• 5. Must be able to perform any physical requirement of the position which includes lifting dead weight or unassisted weight of 50 pounds.
• 6. Is capable of following oral and written instructions.
• 7. Have training, knowledge and experience which allows for general supervision of responsibilities.

REPORTING:

The Speech Language Pathologist reports to the Director of Rehabilitation.

JOB DUTIES:

• Basic knowledge of physical medicine, speech therapy modalities, anatomy and physiology.
• Treat patient in accordance with Speech Language Pathology Standard of Care and Practice.
• Provides assessment, treatment, and rehabilitative services to patients who have experienced injury or disability disease. Provides residents with exercises related to speech language pathology.
• Adheres to standards of physical medicine practice.
• Ability to observe and evaluate treatment effect, discusses observations with staff, physician and Director of Rehabilitation.
• Responsible for the monitoring and documentation of resident treatment and response.
• Assists with formulation of a teaching plan based upon identified learning needs and assesses effectiveness of learning, family is included in teaching as appropriate, staff and physician are apprised of conclusions.
• Meets current documentation standards and policies.
• Adheres to facility’s Policy and Procedure.
