Role Overview
Responsible for providing primary physical therapy care to home health patients across multiple counties, supervising therapy staff, and coordinating interdisciplinary services to ensure optimal patient outcomes.
Key Responsibilities
- Conduct assessments, develop and revise individualized care plans, manage patient progress, and perform routine and complex therapy procedures in patients’ homes.
- Supervise PTAs and Home Health Aides, ensuring quality care delivery throughout the episode of care.
- Provide patient and caregiver education to promote independence and self-care.
- Communicate effectively with physicians, team members, and patients, and document all care accurately following regulatory guidelines.
- Participate in performance improvement initiatives, staff training, and meet productivity standards.
Qualifications & Skills
- Graduate of an accredited physical therapy program.
- Valid Physical Therapist license, BLS certification, and Georgia Driver’s License with insurance.
- Minimum of 1 year of full-time PT experience, preferably in home health.
- Strong assessment, rehabilitation, communication, and organizational skills.
- Proficiency in home health operations, regulatory compliance, and documentation, with OASIS knowledge preferred.
