Responsibilities

• Develop professional and clinical skills through attendance at staff development meetings, educational activities, and professional affiliations.
• Admit new patients according to departmental policy and perform comprehensive initial evaluations for those requiring physical therapy.
• Conduct home safety evaluations and communicate specific needs to the physician and appropriate team members.
• Establish a plan of care with goals based on the patient’s current level of functioning and potential for improvement, in conjunction with the patient, family, caregiver, and physician’s orders. Implement the physical therapy assessment and care plan, continuously evaluating the patient’s response and modifying as needed. Report pertinent observations to the attending physician and adjust the patient care plan as directed.
• Instruct patients and families in the physical therapy program, including the use of prosthetic, orthotic, and assistive devices.
• Participate in the team process, through cooperative efforts with team members, in developing the patient care plan and attending team conferences.
• Supervise and plan home health aide activities in the absence of other healthcare professionals, according to individual patient care requirements and department policy.
• Demonstrate effective communication skills with all team members, patients, and families.
• Maintain clinical record information in accordance with established standards and within established time frames.
• Participate in the review process, agency performance improvement program, orientation, in-service, and staff meetings as scheduled.
• Display flexibility in accepting assignment changes and patient care responsibilities, including participation in weekend coverage and call rotation.
• Other duties as assigned.

• Knowledge in the identification, prevention, remediation, and rehabilitation of patients who have impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease, or other causes. (Required)

Pay Range

$34.71 - $64.55
