Physical Therapist - Home Health

Bring compassionate care directly to patients where they feel most comfortable — at home. We’re seeking a dedicated and independent Home Health Registered Nurse (RN) to provide high-quality in-home patient care while supporting recovery, wellness, and independence.

What You’ll Do:

• Conduct comprehensive patient assessments and develop individualized care plans
• Deliver skilled nursing care and monitor patient progress in the home setting
• Educate patients and families on treatment plans, medications, and ongoing care needs
• Coordinate care with physicians and interdisciplinary healthcare teams
• Maintain accurate and timely clinical documentation and patient records
• Participate in on-call rotation as assigned based on operational needs

What You’ll Bring:

• Active Registered Nurse (RN) license in the state of practice
• CPR/BLS certification required
• Valid driver’s license, reliable transportation, and current auto insurance
• Home health or acute care experience preferred
• Strong clinical judgment, communication, and patient education skills
• Ability to work independently and manage patient caseloads effectively
