About the Opportunity

We're seeking an experienced Physical Therapist (PT) for a permanent Home Health opportunity serving patients throughout the West Valley of Phoenix, Arizona .

This position offers an excellent combination of competitive compensation, independence, and meaningful one-on-one patient care. You'll work directly with patients in their homes to improve mobility, strength, safety, and overall independence while collaborating with a supportive interdisciplinary team.

Responsibilities

• Perform comprehensive in-home Physical Therapy evaluations and reassessments.
  
• Develop and implement individualized plans of care.
  
• Provide therapeutic exercise, gait, balance, mobility, and functional training.
  
• Help patients improve safety and independence in their home environment.
  
• Educate patients and caregivers on home programs, fall prevention, and safety.
  
• Communicate patient progress and changes with physicians and interdisciplinary team members.
  
• Maintain accurate and timely clinical documentation.
  
• Effectively manage an assigned Home Health territory and caseload.
  

Qualifications

• Graduate of an accredited Physical Therapy program.
  
• Active Arizona Physical Therapist license in good standing.
  
• Home Health experience required.
  
• Current BLS/CPR certification.
  
• Valid driver's license and reliable transportation.
  
• Strong clinical judgment and ability to work independently.
  
• Excellent communication, organization, time-management, and documentation skills.
